How your data is used

As part of your treatment it is necessary to record a certain amount of information. This will be in the form of paper based initial and treatment notes. Data is also held on a database, spreadsheet for accounting purposes and occasionally an audio recording is made. All this information including that received by email and telephone on your initial approach to me is confidential and will not be passed to any third party without your specific permission and then only for the purposes of helping your treatment progress.

These data will be stored securely and at some point in the future will be destroyed or deleted. As patients may request further treatment a year or so after I will retain information for at least this period of time. In the event that I cease to practice all data will be destroyed or deleted except that held as part of my record of accounts the retention of which is dictated my HMRC.
